- you are requesting an extension of the period of stay with a long-term visa
- you are requesting a long-term residence permit or an extension of its validity

- you are requesting a certificate of temporary residence on the territory (you are an EU citizen)
- you are requesting a temporary residence permit of an EU citizen family member or a residence card extension
- you are reporting the change of the place of stay on the Czech territory or any other change related to your stay in the Czech Republic
- you are requesting a confirmation of residence
